What is a Community Agent
My name is Brigitte and I am the new Community Agent for Chirk. I have just retired from District Nursing in Wrexham and Flintshire and taken up this post at the end of last year.
As a Community Agent my role is to work with people over 50 living in the local area supporting with:
· Signposting to services and organisations that will support you with health, social and financial needs. Enabling you to feel more independent by focusing on what is important to you.
· Assistance with form filling, blue badge applications, bus passes etc. and signposting to other services that can complete other important applications.
· Signposting to local activities and groups and supporting residents to remain socially active and engaged with their community.
· Helping residents feel more secure and cared for, and to have a better quality of life.
Although I work from home (administration) I am currently in Chirk Library every Wednesday morning from 10-12, I am also able to access confidential space in Caffi Wylfa or Chirk Community Hall at Craig Close. I am also happy to visit residents in their own home if this would be easier.
Please feel free to contact me if you think I can help! 07383 579824 or chirkcommunityagent@avow.org
Feel free to use whichever bits suit. If you have been working on a different site that I have missed let me know.
Kind regards
Brigitte
